REQUIREMENTS FOR PRE-SCHOOL ENROLLMENT

You must provide the following:

• Certified Birth Certificate
• Current Immunization Card (blue form DH680 7/2001)
• Current Health Certificate (physical within last 12 months- Form DH 4030) and
• Parent and/or Guardian must be a West Area Adult School student

Program Requirements

• Age 3-5 years old
• Fully potty trained
• Parent provides lunch
• Parent participates in a weekly “Parent and Child Time” PACT activity
